Yizkor Book Donation

The OJC Book of Remembrance contains readings, the Yizkor prayer, and the names of deceased loved ones whom our congregants wish to honour. It will be published for the first Yizkor service of the year, at Yom Kippur.

Inclusion in the memorial book is by donation to help defray the costs of printing.

All names must be received by September 1st

In the Additional Notes section of the donation form, please provide:

• The full name(s) of the person(s) you wish to recognize
• The name(s) of those making the dedication
